1. Where do I find the claim forms for my Blue Cross Health and Dental Insurance?

On the PARA website under:
Resident Quarters > Frequently Used Forms > General Residency Forms


2.  What if a dentist doesn't take Blue Cross?

If I dentist doesn't take Blue Cross you will need to pay the full amount, then submit a claim for reimbursement of the amount covered by Blue Cross.
